Cellular Immunology Proficiency Testing Program

Proficiency Test Summaries

Please note: The Malignant Immunophenotyping proficiency test form for the electronic submission of cell markers can be accessed by clicking here. Please refer to the instructions below when submitting the forms.

  • Open the CD marker report form
  • Save the report form to your computer
  • Complete one form for each sample received. The four digit PFI number for your laboratory, the sample ID, and the sample name must be on each form to ensure the form data is correctly downloaded. The sample ID and sample name are listed on the sample tube. The options for each marker are listed in a dropdown format next to each marker; the default value for each marker is "Not Analyzed". This option will be submitted unless another is chosen.
  • At the bottom of the form please add any markers not currently listed. In addition, state the lineage and stage of the aberrant population in the appropriate section. Any comments can be added as well at the bottom of the form.
  • After completing each form, save the form using the following extension:
  • 11-02MLIMPT_(Sample ID)_(PFI#)

    As an example, if a lab with the PFI 0000 fills out a form for sample 11102501, the file extension for saving the file would be 11-02MLIMPT_11102501_0000.

  • E-mail the completed forms to Nancy Andersen at njo02@health.state.ny.us by the proficiency test due date; to view due dates for each PT event please see the current year proficiency testing schedule.
