This slide was taken from a peripheral blood smear of a 42 year-old male with a history of leukocytosis and anemia. Upon admission, a white blood cell count of 134.0 X 103/µL was noted. Further testing revealed that the patient was positive for Philadelphia chromosome. Diagnosis was accelerated phase of chronic myelogenous leukemia (CML). The arrows indicate two promyelocytes with round, eccentrically placed nuclei and with light blue cytoplasm containing prominent reddish purple granules. However, because it is the policy of some laboratories to identify the more mature cell when in doubt, the answer of myelocyte was also accepted.
