The correct response for the arrowed cell is Pelger-Huët (Code 22). This slide was taken from a case of a patient diagnosed with Pelger-Huët anomaly. Both of the cells seen on this slide are bi-lobed, the nucleus has failed to properly segment. While by definition they are segmented neutrophils, when the majority (or in this instance two cells in one field) appear bi-lobed, a diagnosis of Pelger-Huët anomaly is appropriate. The neutrophils of this benign anomaly appear to function normally.
