- Place the primary specimen collection vessel into the bubble wrap pouch.

- Roll the bubble wrap around the vessel.

- Insert the bubble-wrapped specimen vessel into the plastic bag containing an absorbent pad and seal the bag.

- Wrap the completely filled out laboratory requisition (history form) around the outside of the plastic bag with the bubble-wrapped specimen vessel inside. DO NOT pack the requisition inside the plastic bag with the specimen.

- Insert the properly wrapped specimen into the white plastic container. Approximately 1/2" of the wrapped specimen will protrude from the end of the white plastic container. Do not fold the excess 1/2" of material inward.

- Place the cap on the container by aligning the peg on the cylinder with the slot on the cap, push together, and twist clockwise to seal.

- Assemble the pre-addressed cardboard shipping box and the top and bottom cardboard inserts. Place the bottom cardboard insert (4 hole side up) into the box. Holding the sealed white plastic container upright, insert it into one of the 4 vessel holders located in the bottom of the box. Align the top cardboard insert (4 hole side down) on top of the containers.

- Close the top flap of the shipping box 9. Send box to the laboratory via your and secure the top. Labeling carrier. requirements may vary with different carriers. Please check with your carrier and refer to sampling instructions.

- Send box to the laboratory via your carrier.
