- Place a pure broth culture of at least 3ml into a labeled freeze vial (not provided by Wadsworth Center). The top should be sealed tightly and wrapped with parafilm.
- The labeled aliquot should be placed inside the large 50ml screw cap tube.
- The screw cap tube should be placed in a "Ziploc" storage bag with an absorbent pad.
- The 'Comprehensive Mycobacteriology' form should be completed and rolled around the plastic bag, (do not place the form inside the bag) and placed inside the storage container with the locking screw cap.
- The locked tube should be vertically placed in the pre-addressed and assembled box and sealed.
- Do not include specimens not intended for the Clinical Mycobacteriology Laboratory.
- Seal the box and be sure to mark it appropriately so that it complies with the standards of your courier.
