Medical Home Work Group
Vision: A medical home for all individuals with special health care needs that encourages/supports partnerships among patients and parents, health care and public health professionals, and the community to navigate the healthcare system with the goal of helping the individual to reach his or her full potential.
Goal 1: Work institutionally with all specialty departments to determine what a medical home means and what their roles and responsibilities are in co-managing patients.
Goal 2: Develop strategies and support to assist providers in obtaining maximum reimbursement for services related to medical home activities - support providers’ participation in medical home improvement.
Goal 3: Support families to become informed and empowered consumers and partners.
