Transition Work Group

Vision: Every young adult with special health care needs can make a seamless transition into adult health care.

Goal 1: Identify/develop and pilot models of transition for children with special health care needs from pediatric to adult medical care.

Goal 2: Ensure that people with special health care needs and their parents/guardians are able to assume levels of responsibility appropriate to abilities of the people with special health care needs.

Goal 3: Provide a national leadership role to develop and nurture the National Metabolic and Genetic Conditions Transition Work Group.
