Laboratory Director Requirements

Laboratories performing non-waived testing must employ a laboratory director that meets specific education, training and experience based on laboratory testing complexity. Documentation of laboratory director qualifications must be submitted with any new application or upon notification of a change in laboratory director to POLEP.

Acceptable forms of documentation for laboratory director qualifications:

  • Copies of Diplomas or transcripts from an accredited institution
  • Board Certifications
  • Laboratory experience documented via employment records or letters of recommendation
  • CME certificates from approved online laboratory director training programs (moderate complexity testing only)

Individuals who attended foreign schools must have an evaluation of their credentials determining equivalency of their education to education obtained in the United States. Failure to submit this information will delay the processing of your application.
